Can you please help me?
When i move the model from the center point and pose it, suddenly the bones split up.
it makes the model's hands and legs extended weirdly like a ghost (;w; )
Can you help me how to solve it?

Oh, for those who are using blend ver2.74 and can't found the pose mode and the bones.
You can select the model then open the properties panel by pressing N button and scroll down on it
There'll be a "MMD Display" section. Open the section, check the armature box, and you'll be able to see the armatures(bones) and change it to pose mode.
I hope this can help Ouo

Ok, I have had this problem for awhile and just recently found a fix. See if this works.

NOTE: DO NOT press "build" in the "MMD Tools" tab!!! (It makes it impossible to move the model off origin)

1)Activate the armature (see tutorial below, a couple of posts down), but leave the bone shape Octahedral. Go into pose mode (Click where it says "Object mode"(under the picture of the model) with the armature selected)
2) Select the bottom-most bone in the middle of the model like in this image i.imgur.com/TFHgdMV.png (the model looks weird because the model was "built")
3) Move that bone to move the model off the origin!
4) Feel free to pose! (Check out the For Easier Posing section of my tutorial) You can make the bones "stick" shape now.

Let me know if this helps.

Hey there! I submitted my work Blendistory (9/5/2015) to a Blender Nuts contest in an attempt to win some prize money that I could use to create my own contests for my group BlenderTutors. However, it looks like I'm currently tied for third. Could you please vote for me? The voting site is CONTEST VOTING, I'm Entry #7. If you know anyone else who could vote please ask them too! Thanks, have a nice weekend!:D (Big Grin)

No problem. I'd love to help! Blender is an awesome animation software, free, and open-sourced. The more you use it the easier it gets. I'm still learning, but I'll see what I can do. Been at it for a few years now. Good luck!

Follow my tutorial below this post (copied below), make certain the armature/bones are selected when attempting to enter pose mode:

MISSING BONES? : If your model's bones disappeared, 1) select the model, 2) Select the "MMD Tools" Tab on the far left. 3) Scroll down to the "MMD Model Tools" Section. 4) Check "Show Armature" and the bones should appear if properly created in MMD.

For Posing: 1) Select the armature (bones), go to the right and under the list of objects in the scene there is a bar (Starts with a picture of a camera). 2) Make sure the  picture of the person is selected (it should be if you selected the armature.) 3) Under "Display" Select "Stick" and check "X-Ray. 4) Go into pose mode (Click where it says object mode with the armature selected) 5) begin posing! (Select a bone, press r to rotate, g to "grab" or translate.)

Under the 3D view (where you see the model) there is a rectangle that says "object mode" or "edit mode" next to that (on the right) is a button with a sphere on it. Click on it and then on the one labeled material (black and yellow)
